Specific Collection Guidelines—Stool Collection—Timed Specimens (24, 48, 72 Hours)

Instructions to the Patient

Eat a normal diet containing 50-150 grams of fat per day for at least three days prior to beginning fecal collection. This diet should be maintained throughout the collection period. Ideally, no medications should be taken immediately prior to or during the collection period. Exclude substances like castor oil and mineral oil from the diet.

Instructions to the Lab

  • Only specimen portions of less than 500 grams per container are acceptable for transport. Paint cans are NOT acceptable and will be rejected and discarded. Collections with a total weight of >500 grams must be processed by the laboratory as described below and a 20 gm aliquot submitted.
  • Alternatively, multiple approved individual collection containers (one or two per 24 hour period), each containing less than 500 grams per container, may be submitted. Each container must be appropriately labeled and must be further noted as 1 of 3, 2 of 3, 3 of 3, etc.
  • Preparation of aliquot for timed stool collection
    • Weigh the entire stool specimen and record the total weight. Only specimen portions of less than 500 grams are allowed. Paint cans are not acceptable; these specimens will be rejected and discarded.
    • Transfer the entire collection to a blending container.
    • Write patient name legibly on specimen container. Separate containers/tubes should be used for each test ordered on the same patient, especially for frozen specimens.
    • Using a graduated cylinder, add sufficient water to give “milk shake” consistency. The amount of water added will depend on the consistency of the specimen. This usually requires a water-to-feces ratio of one-to-one. Do not add water to a liquid specimen.
    • Record the amount of water added, blending feces and water until completely mixed.
    • Pour a 20 g (20 mL) aliquot into a plastic container and transport frozen. Minimum: 10 g (10 mL). Use ARUP Supply #040910, unpreserved stool vial. Do not send the entire stool collection.
    • Record the total weight of the collection, the amount of water added, and the length of the collection period on both the test request form and on the aliquot container.
